Creatively Wild requires anyone who attends our in-studio classes to register and complete payment prior to class.If you have not registered through our website, you will need to fill out the below waiver, this is in the case of:

If you arrive at the studio hoping to register for a class. Even if you have been accepted into the class and pay cash, you still need to fill out our waiver.
If you are registering using a gift certificate as payment
If you are registering through a third party platform (CourseHorse, KidPass)
If you have already registered and paid, it is not necessary to fill out this form as you will have already completed it as part of our regular registration process.

COVID Waiver

By registering for this program, I AGREE & ATTEST to all the following, on behalf of myself named herein as a participant in this program or any minor participant (s) mentioned herein that I represent and am legally responsible for. I am aware of the risk of Covid-19 associated with participating in any in-person experience, including all Creatively Wild Art Classes & Programs I will NOT attend this program mentioned herein if I am currently experiencing symptoms of fever, fatigue, difficulty breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease, or have experienced/exhibited any of these symptoms within 14 days of this event. I AGREE on behalf of myself as the participant or participants named herein to agree to hold harmless, release, waive and indemnify Creatively Wild Art, Inc, its members, employees, agents, successors and assigns to any & all injuries, illness, medical conditions, claims, liabilities, damages, expenses or judgments from participation in any and all Creatively WILD's programming and/or activities, including use of the area(s) and/or equipment, participation in a class or program at Creatively Wild Art, Inc. Any defense, cost or expense arising out of any and all claims, injuries, liabilities or damages arising out of or related to participation in any and all of our programming, as set forth, including attorneys' fees. I AGREE NOT TO ATTEND this program if I as a participant or any minor participant(s) mentioned herein, or any member(s) of my household, have been diagnosed with COVID-19 within 14 days of this program. I CERTIFY the information that I have entered herein to be accurate and true. I am at least 18 years old and I have read and agree to the terms of this agreement. I CERTIFY that I, & all other participants named herein in this form do NOT currently have Covid-19. I CERTIFY that I, & all other participants named herein am not currently living with anyone who is suffering or recovering from Covid-19 or any symptoms related to it. I CERTIFY that to the best of my knowledge, in the past 14 days, I or the above minor mentioned herein above have not been in close contact ( or within 6 feet for at least 10 minutes) with anyone who tested positive for COVID-19 or who has or had symptoms of COVID-19? I AGREE + CERTIFY that I or any participant(s) mentioned herein are expected to stay away from any in-person class at CWAS if I develop any COVID-19 symptoms or flu-like symptoms, and that I will notify the studio of my situation immediately if this should happen.

Art Supplies

Most materials are provided but we do NOT supply a CANVAS for adult classes. Please bring a canvas with you to class. We recommend a medium size canvas to start. Keep in mind, you may need multiple canvas' during a full class course. SPECIALITY MATERIALS like glazes or finishing mediums are NOT supplied. If you require speciality materials please bring them with you to class. We recommend purchasing at your neighborhood supply store (support local business) or you can order online if you prefer.

Rescheduling or Canceling

Cancellation + Rescheduling Policy: To reschedule a class, a Rescheduling Ticket must be purchased at least 24 hours in advance. If you fail to notify us in time (at least 24hrs), you are not entitled to any make-ups, rescheduling, credit, or refunds. PLEASE NOTE during the ongoing crisis, if a class is canceled due to situations beyond our control, CWAS will not be issuing any refunds. Instead, you will be offered the option of transferring to an ONLINE experience. We will also offer you the option of credit valid for six months from the date of purchase and to be used on any classes or activities. You may also transfer this credit into a Gift Certificate to gift to someone if you prefer.

Photo Release

CWAS may take photographs or videos of artwork or work in progress at the studio during classes which may/ or may not be used for publicity purposes. These photos may be published on our social media pages or platforms &/or website, &/or may be posted on other promotional materials or in other promotional media. NO STUDENT will be identified in these photographs or tagged/identified in any photo without their consent. Please note, we archive all work created at our studio. If you do not wish your child to be in a group photo, please state this clearly in the notes section of this form. All photographs are the property of CWAS and we do not allow these photos to be published by third-party or by any other media or source. We value & are protective of our students, their interests & privacy.
